How do I refer a patient?
There are two easy ways to refer a patient.
- You can contact us directly by phone, fax or email with the patient's information and we will contact the patient.
- You can provide the patient with a business card or referral slip and tell the patient to contact us themselves.
Will I receive any feedback?
As part of the health privacy act, each of your patients will be asked to sign a confidentiality agreement which gives us the ability to transfer information back and forth between you and Revive Nutrition Consulting Inc. We feel very strongly about a team approach and therefore place significant importance on communicating with you what our nutrition assessment is and the advise provided to your patient. Every consultation will be followed up with a letter to you.
Are your services covered by Insurance?
Basic Alberta Blue Cross insurance does not cover the services of a private practice registered dietitian. If a patient has a health spending account with their Blue Cross coverage, services are provided through this account. Other private insurance plans do cover services of registered dietitians. If your patient has private insurance coverage encourage him/her to call and see if our services are covered.
Can I ask you a question?
Absolutely! If you would like to ask us a question or discuss a patient with us, please do not hesitate to contact our office. We also offer nutrition seminars for you and your staff on a nutrition topic of your choice.
I would like to meet you before I refer a patient, is this possible?
We are available to come to your office and meet you and your staff. We will provide you with business cards, referral slips and a healthy nutritious snack! If you are interested in meeting us, please call our office.
